2. In-Home Visit

Next, we schedule a convenient home visit with you, your family, and the loved one who may receive care. During this visit, we assess the home environment, discuss daily routines and safety needs, and explain how our personal care and supportive home care services can help your loved one remain comfortable and independent at home.

3. Customized Care Plan

Based on what we learn, we design a personalized care plan focused on the right mix of personal care (such as assistance with bathing, dressing, grooming, and hygiene), supportive home care (like light housekeeping, meal preparation, and errands), and private care services tailored to your family’s preferences and schedule. We review this plan with you in clear, simple language so you always know exactly what to expect.

4. Carefully Matched Caregivers

We thoughtfully match your loved one with a caregiver whose skills, experience, and personality align with their needs—whether that means more hands-on personal care, consistent supportive home care, or flexible private care services. In line with state regulations, eligible family members may also be able to serve as caregivers, allowing your loved one to receive care from someone they already know and trust. If a change is ever needed, we respond quickly to ensure you feel confident and comfortable with the caregiver in your home.

5. Ongoing Support and Oversight

Our team stays actively involved, reviewing caregiver notes, checking in with your family, and adjusting the care plan as needs change. You are never on your own—Alliance Total Care provides continuous support and oversight, so your loved one receives safe, reliable, and compassionate care day after day.
