Position Summary
As our Part-Time RN, you will partner with our Home Care Agency and Physician's Office to develop and oversee personalized care plans that help clients safely remain in their homes while maintaining their independence and quality of life.
Your work will directly contribute to improving client outcomes and supporting their healthcare journeys through compassionate, high-quality care.
What You'll Do
- The RN manages and ensures that clients receive high-quality supportive care; personal care, and advanced personal care are provided as described in the client's Plan of Care.
- Conduct initial assessments, re-assessments, and supervisory visits every 50-60 days with clients based on the DHS guideline 105.17.
- Develop individualized Plan of Care based on the client needs and preferences.
- Delegate nursing tasks to CMS staff and supervise the work to ensure client’s home care services are met according to the Plan of Care.
- Train and educate caregivers on:
- Direct care of competency
- Standard precautions
- Infection control
- Timesheet submission, including Electronic Visit Verification (EVV).
- Monitor care quality and ensure compliance with agency policies and state/federal regulations
- Stay current on home care best practices and healthcare standards
Qualifications
Required:
- Current Registered Nurse (RN) license under s.441.06, Stats.
- Reliable transportation and current auto insurance
Preferred (but not required):
- Previous experience in home care or personal care services (we're happy to train the right candidate!)
- Experience with Microsoft Office 365 is a plus
Work Environment & Physical Requirements
This role combines office and field-based responsibilities. Travel to client homes is required for assessments and evaluating caregiver performance. Candidates should be comfortable:
- Traveling between office and client locations
- Navigating stairs
Stooping, kneeling, reaching, and demonstrating caregiving techniques when needed
Compensation & Schedule
Position: Part-Time Licensed RN - Contracted
Schedule: Approximately 25 hours per week
Pay: Paid based on experience
Sign-On Bonus: $3,000.00
Travel mileage reimbursement

We are continuously hiring qualified caregivers for both part-time and full-time positions. If you have a loved one who needs Personal Care or Supportive Home Care services, you may enroll them with our agency and assist with their activities of daily living (ADLs). Caregivers must be at least 16 years of age and may not be a minor, spouse, or legal guardian of the client. All Personal Care Workers (PCWs) are required to complete our hiring process and successfully complete training prior to providing in-home care.
