Positions Summary

Registered Nurse

The RN works with Physician’s office to develop a plan of care for each Patients to achieving his/her healthcare needs. The goals are to prevent self-neglect, improve physical functions, promote independence and be close to family members for as long as possible before moving to a caring facility. The RN conduct initial assessments and evaluate Patient’s personal care needs according to State regulations. The RN also provides training and supervises Personal Care Workers (PCW) to ensure that care is being provided according to Patient’s plan of care. The RN reports directly to the Care Manager and/or Administrator. PT contracted positions available.

Personal Care Worker

A Personal Care Worker also known as a Caregiver, works under the supervision of an RN Supervisor to provide care necessary to the Patient’s physical and mental condition in order to maximize the Patient’s health status. Tasks performed will include but not limited to hygiene care, activities of daily living (ADL), and other activities specified in the Patient’s plan of care. Job location: Milwaukee

Office Assistant

The Office Assistant provides exceptional customer services, handling incoming phone calls, collecting, and reviewing employee timesheets, assisting employees with EVV (Electronic Visit Verification), preparing personnel files and performs other tasks as assigned by the Manager. Job location: Milwaukee

Desktop Software Support Specialist – Internship Only

We are offering an internship position for students who wish to gain experience in modern office technology. Students must have exceptional skills in Microsoft Office 365 applications to be considered for this position. Under the supervision of the Manager, students will gain experience providing desktop software support and technical assistance to assist staff in organizing work files, sharing documents and communication systems throughout the organization. The goal is to increase staff efficiency and productivity. Job location: Milwaukee

Prenatal Care Coordinator- Bilingual

We’re hiring for a Prenatal Care Coordinator-Bilingual candidate who has experience coordinating services for at- risk or low-income pregnant women. The individual candidate must have at least two years’ experience in community health services or in volunteer capacity related to health education and nutrition counseling. Experienced working with a diverse ethnic population, Women, Infants and Children Supplemental Nutrition Program (WIC), an Early Identification of Pregnancy program or the local high school is a plus.
